I request that my child be registered as a prospective pupil for 11+ entry. I understand that the Registration Fee of £400 is non-refundable and that Wellington College Prep and Wellington College (through the Master, as the person responsible) may obtain, process and hold personal information about my child, including sensitive information such as medical details, and we consent to this for the purposes of assessment and offer process. I also understand that submitting this form does not give rise to a contract with the school and that offers of places are subject to availability and the admission requirements of the school at the time of offer.
